It does appear that the pain is too unbearable for him.
I take it that the spade symbol at his chest represents the core of his feelings?
Also, it is interesting that the lines around the symbol look like the number 8, when 8 is considered a 'lucky number' for some.
Now, the longer I look at the spade it looks like a lighted torch, mainly because of the hints of orange colour.
